Immunization Requirements for Kindergarten

4 DPT - If the 4th is done before the 4th birthday, a 5th is required.
3 Polio - If 3rd is before 4th birthday, a 4th is required.
2 MMR
Hepatitis B - 3 shot series = 1st and 2nd shot, 1 month apart; the 3rd of the series completed ~5 months later, within 6 months.

These shots have to be obtained before school or be in process. If not, it may result in exclusion from school until obtained.

This is in accordance with the state law amended Jan. 15th, 1999.

At registration you will be given the forms for a medical and dental check-up. Please call our office (440-233-7113) if you have any questions.

Please bring shot records to school when registering. They must be in the office no later than the second week of September.
